What Are Club Drugs? Definition, Examples, Risks & Treatment

Club drugs refer to a wide array of illicit drugs and substances being utilized recreationally in social and party-type settings. This variety of psychoactive drugs alters the user’s mood, perception of reality, energy level, and cognitive functions (producing desired changes to the user). 

The classification does not involve any specific chemical compound. Rather, it encompasses multiple types of psychoactive compounds, including stimulants, dissociative anesthetics, hallucinogens, CNS depressants, etc., based upon their usage patterns and locations. This diversity is part of what makes club drugs particularly challenging to address from a public health perspective. The risks vary significantly from one substance to the next, and the populations affected are equally varied. 

Not everyone who uses club drugs will develop an addiction, but the risk exists across all of these substances. Substance use disorders exist among users of MDMA, ketamine, methamphetamine, GHB, and roofies, each through different neurochemical mechanisms. Stimulants like MDMA and methamphetamine create dependence by flooding the brain with dopamine and serotonin, which over time makes it difficult to feel pleasure, motivation, or emotional well-being without the substance.

Dependence on central nervous system depressants such as GHB/Rohypnol develops rapidly among users, with severe medical implications for withdrawal from either substance.[2]

Examples of signs indicative of club drug addiction include continued use regardless of undesirable consequences, continued time devoted to acquiring or recovering from the drug, using more than intended, and experiencing cravings/withdrawals when abstaining from the substance.

List of the Most Common Club Drugs

Below is information about the most commonly encountered club drugs. Additionally, we describe how each substance works, as well as the dangers associated with each substance.

MDMA (Ecstasy/Molly/Adam/XTC)

MDMA (methylenedioxymethamphetamine) is a man-made psychoactive substance. The primary mechanism of MDMA is to act as both a stimulant and a mild hallucinogen. MDMA is sold in two main forms: pressed pills or tablets, and powder or capsules. There are numerous street names for MDMA, including ecstasy, molly, Adam, and XTC.  Users report feelings of euphoric excitement and increased energy after consuming MDMA. The drug’s stimulant effects, which increase energy and reduce inhibition, helped drive its popularity in rave and dance club settings where extended physical activity and social engagement were central. However, MDMA has severe risks. MDMA rapidly elevates a user’s body temperature (hyperthermia), creating an extreme danger if consumed in excess amounts in warm/crowded areas. MDMA also significantly increases a user’s heart rate and blood pressure, placing additional stress on a user’s cardiovascular system.[3] 

In addition, MDMA reduces the availability of serotonin in a user’s brain. Prolonged MDMA use depletes serotonin over time, which can lead to lasting mood disturbances, cognitive difficulties, and memory loss. In reference to molly or ecstasy, it is very common for what is sold with this name to contain other active ingredients besides pure MDMA. Each dose therefore contains unknown quantities of other substances, posing varying degrees of risk.

GHB (Liquid Ecstasy/Georgia Home Boy)

GHB (gamma-hydroxybutyric acid) is a central nervous system depressant that occurs naturally in the brain in very small amounts as a byproduct of the neurotransmitter GABA. However, GHB can be synthesized and manufactured illegally in clandestine labs and then marketed and sold as a clear liquid or white powder. The street names for GHB are liquid ecstasy and Georgia Home Boy, and despite sharing a street name with ecstasy, GHB has no chemical relationship to MDMA. GHB was briefly approved for prescription use to treat narcolepsy via the brand name Xyrem, providing GHB with a legitimate medical history that contributes to misunderstanding its widespread misuse.

GHB is extremely hazardous to consume because there is only a small differential between what constitutes a recreational dose and a lethal dose. At low doses, GHB may produce feelings of euphoria and relaxation. At higher doses, it rapidly causes drowsiness, confusion, loss of consciousness, and respiratory depression.[4] Since GHB is almost completely undetectable in terms of appearance, color, or taste, it is often administered surreptitiously in beverage containers for purposes of committing drug-facilitated sexual assaults, earning GHB the status of a date-rape drug. When combined with alcohol/CNS depressants, GHB can induce rapid loss of consciousness/death.

Ketamine (Special K/Vitamin K)

Ketamine is an anesthetic agent used both clinically in humans and in veterinary medicine. Outside of clinical settings, ketamine goes by the street names Special K and Vitamin K. When misused, ketamine creates dissociative effects, causing users to become detached from their surroundings/environment and detached from their bodies. High doses create what is referred to as a k-hole, a profound state of dissociation and near immobility lasting approximately one hour.[5]

Ketamine affects blood pressure, impairs memory, concentration, and motor coordination, and due to its dissociative effects, significantly increases the risk of accidental injury while under its influence. Similar to other dissociatives or PCP-related substances, chronic misuse can lead to psychological dependence. Severe bladder and urinary tract damage resulting from chronic misuse has been reported uniquely associated with ketamine, requiring surgical intervention in severe cases.

Rohypnol (Flunitrazepam/Roofies)

Rohypnol is the trade name for flunitrazepam, a potent benzodiazepine with action profiles that are approximately 10 times stronger than Valium. Rohypnol acts similarly to other benzodiazepines, depressing the CNS and thereby inducing sedation, muscle relaxation, and amnesia. Flunitrazepam is not approved for medicinal use within the U.S., although it is used medicinally in some foreign countries. Within the U.S., flunitrazepam is listed as a Schedule IV controlled substance and is prohibited from production, distribution, and personal possession. 

Rohypnol has gained notoriety as a date-rape drug due to its ability to render victims incapacitated and rapidly unconscious with memory loss, thereby enabling perpetrators to commit crimes against victims without fear of resistance.[6]

LSD (Acid/Lysergic Acid Diethylamide)

LSD (lysergic acid diethylamide) is a hallucinogenic psychedelic with a high potency level that was created synthetically from a fungus found on grain crops. Hallucinogens distort sensory input and alter thought processes, thereby allowing users to perceive vivid visual and auditory hallucinations lasting 8-12 hours. 

LSD continues to be frequently found at raves and festival events. While LSD is not considered physically addictive in the way stimulants or depressants are, users can experience serious psychological distress from long-term use, including hallucinogen persisting perception disorder (HPPD) or drug-induced psychosis depending on individual vulnerabilities.

Club Drug Effects on Health

Many of the short-term effects experienced by users of club drugs are varied among each substance. Across the category, many club drugs share common short-term effects including altered perception of time and surroundings, lowered inhibitions, impaired judgment, and reduced coordination. These symptoms not only account for the health-related risks posed by club drugs individually but also create circumstances for users to make detrimental decisions while under the influence of these substances, such as participating in unsafe sex, driving while intoxicated, or being vulnerable to assault. Longer-term health consequences of club drug abuse can include impaired memory, mood disorders, cognitive decline, and organ damage. Chronic misuse of methamphetamine, GHB, and Rohypnol carries the additional risk of severe physical dependence.

Interactions pose another risk; many users choose to use club drugs alongside other substances, drastically increasing the possibility of overdoses, emergency room visits, and severe medical complications. Club drug use imposes a tremendous healthcare burden, including emergency room admissions, hospitalizations, and deaths.
