Understanding Crack Addiction: What It Does to the Body and How Recovery Is Possible

Crack is a type of cocaine that has been converted into a crystallized solid using baking soda or ammonia and water. Instead of being administered via snorting like powdered cocaine, crack is smoked. The name “crack” was given due to the distinctive crackling sound created when crack is heated. Sometimes crack is referred to by various street names, such as “rock,” “base,” or “freebase.”

As a result of its high potential for abuse and dependency, crack has been designated as a Schedule II controlled substance under the U.S. Controlled Substances Act.[1] As a stimulant, crack falls into a different category than opioids and amphetamines; although similar to these other two categories, crack has a profound effect on the brain’s reward system.

Powdered cocaine has been abused as a drug for decades. However, the widespread emergence of crack occurred in the late 1980s as a more affordable and instant alternative to the powdered form.

Knowing how crack affects your brain can provide insight as to why people develop crack addictions so quickly, as well as why quitting without support can be so difficult.

After someone smokes crack, it enters their bloodstream rapidly through their lungs and arrives at their brain in mere seconds.[2] One reason why crack is considered more immediately addictive than its powdered form is because it enters the brain so much faster. When someone smokes crack, it triggers a rapid flood of dopamine in the brain; a neurotransmitter related to feelings of pleasure and satisfaction. After this explosion of dopamine subsides, the user’s dopamine levels plummet, resulting in strong urges to smoke crack again.

Over time, and with repeated exposure to crack, the brain relies upon crack to generate dopamine instead of relying on natural production methods. This disruption to the brain’s reward system is essentially what creates crack addiction and is why stopping on your own can be so challenging.

Other physical effects caused by using crack include:

  • Rapidly increased heart rate and high blood pressure
  • Restriction of blood vessel flow
  • Elevated body temperature
  • Dilated pupils
  • Loss of appetite leading to significant weight loss
  • Pain in chest area or an irregular heartbeat

Young and normally healthy individuals have experienced heart attacks and strokes as a direct result of smoking crack.

Smoking crack puts incredible stress on both the heart and blood vessels.[3] Researchers found that heart attacks and sudden cardiac deaths represent a significant portion of total crack-related emergency room visits.

In addition to cardiovascular issues, the mental health implications associated with prolonged crack abuse are just as dire. Chronic abuse can lead to crack-induced psychosis.[4] Psychosis occurs when a person experiences paranoia, hallucinations, and disconnection from reality. These symptoms can occur even if the individual has never had any pre-existing mental health disorders. These symptoms may also remain present for a considerable amount of time even after the person has stopped using.

Identifying Signs of Crack Addiction

While everyone’s journey with crack addiction appears differently, certain indicators exist commonly amongst individuals struggling with crack addiction. Recognizing these indicators is an important step in identifying when an individual needs to seek assistance.

Signs of behavioral dependence include:

  • Recurrent cravings for the substance
  • Emotional highs or lows, and mood swings between uses
  • Social withdrawal or abandonment of previously enjoyable activities
  • Financial strain from spending money (including vital savings) on crack
  • Paranoia, hallucinations, intense anxiety, or signs of psychosis
  • Acting secretively or disappearing for periods of time

Signs of physiological dependence include:

  • Substantial weight loss or appearance changes
  • Burnt lips or fingers from holding a pipe
  • Dilated pupils or rapid eye movements
  • Respiratory problems, coughing, and breathing difficulties, secondary to smoking the substance
  • Sleep pattern disruptions

What Withdrawal From Crack Looks Like

Another misconception regarding crack is that since it does not produce withdrawal symptoms like those found with opioid or alcohol withdrawal, it cannot be as habit-forming or as hazardous to quit. While true that withdrawal symptoms from crack are primarily psychological in nature and therefore do not have the same degree of physical discomfort as opioid or alcohol withdrawal, this does not diminish the reality that withdrawal from crack is a serious and difficult experience. Individuals who abruptly cease or drastically cut back their use of crack often report experiencing what some refer to as a “crash.“[5] 

Some typical withdrawal symptoms include:

  • Extreme fatigue or sleeping excessively
  • Strong urges or cravings for additional doses of crack
  • Feelings of depression or helplessness
  • Feeling irritable or anxious
  • Restlessness
  • Decreased concentration or memory difficulties
  • Increased hunger or appetite

While crack withdrawal does not carry the same physical risks as opioid or alcohol withdrawal, the psychological experience can still be intense and genuinely difficult to manage without support. The primary reason for relapsing when withdrawing from crack is largely attributed to the intensity of cravings experienced during withdrawal. While crack detox rarely requires the same level of medical intervention as alcohol or benzodiazepine withdrawal, having a clinical team nearby provides important support as the brain begins to stabilize.
